Craft Peptides does not accept, participate in, or bill any of the following:

• Medicare — the federal health program for individuals 65 and older and certain disabled individuals
• Medicaid — the federal and state program for low-income individuals and families
• Private health insurance — including PPO, HMO, EPO, POS plans, or any employer-sponsored health plan
• Medicare Advantage plans — including all Part C plans offered by private insurers
• TRICARE / VA benefits — military and veteran health coverage
• Workers’ compensation — we do not treat work-related injuries
• Health Savings Accounts (HSA) or Flexible Spending Accounts (FSA) — you may use these cards at your own discretion, but eligibility for reimbursement is your responsibility to confirm with your plan administrator

Craft Peptides will not submit claims, superbills, or any billing documentation to Medicare, Medicaid, or any private insurance carrier on your behalf — for any reason, at any time.

We do not provide insurance-compatible billing codes (CPT, ICD-10, HCPCS) for the purpose of insurance reimbursement. Any documentation we provide is for your personal records only and is not formatted for insurance submission.

The treatments and medications available through Craft Peptides fall outside standard insurance coverage for the following reasons:

• Peptide therapy — peptides such as BPC-157, Sermorelin, Ipamorelin, Tesamorelin, TB-500, CJC-1295, and others are not FDA-approved finished drug products and are not recognized as covered treatments by any major insurer or government program
• Compounded medications — compounded drugs are excluded from coverage under Medicare Part D and most private insurance formularies
• Hormone optimization — elective hormone therapy for wellness, performance, or anti-aging purposes is not considered medically necessary under standard insurance criteria
• Wellness & longevity protocols — preventative optimization and longevity medicine are not covered categories under any current government or private insurance program

By enrolling with Craft Peptides, you agree that you are personally and solely responsible for all fees associated with your care, including:

• Consultation and telehealth visit fees
• Compounded medication costs and pharmacy dispensing fees
• Shipping and handling fees
• Laboratory testing fees ordered through Craft Peptides
• Membership, protocol, or subscription fees

All fees are disclosed to you before any purchase is made. You will never be charged without your explicit authorization. Fees are non-refundable once services have been rendered or medications have been dispensed, as outlined in our Return & Refund Policy.

Because Craft Peptides does not participate with any insurance plan, there is no “balance billing.” The price you are quoted is the price you pay — there are no hidden insurance adjustments, claim denials, or surprise bills after the fact. What you see is what you owe.
